Rezumat

Mii de pacienti se prezinta anual in cabinetele de medicina generala sau ORL cu disfunctii chemosenzoriale. Aceste disfunctii pot avea un impact deosebit asupra calitatii vietii mai ales la pacientii a caror viata sau siguranta depinde de functionarea normala a simtului olfactiv sau gustativ. O testare psiho-fizica precisa poate sa fie de mare ajutor in constatarea existentei si evaluarea disfunctiei olfactive. O asemenea evaluare impreuna cu anamneza, examinarea fizica si explorarea radio-imagistica pot sa ajute in diagnosticarea diferentiata a disfunctiei olfactive.

Evaluarea clinica a olfactiei a fost facuta pe un lot de 231 pacienti care s-au prezentat cu diverse acuze chemosenzoriale. Evaluarea de rutina a unui pacient a cuprins anamneza, examen complet ORL, evaluare chemosenzoriala, examene de laborator, examen radiologic. In unele cazuri, pentru stabilirea corecta a diagnosticului etiologic, au fost necesare examene suplimentare de neurologie, endocrinologie, boli interne sau psihiatrie.  Testarea functiei olfactive a cuprins doua parti: test de detectare a pragului si un test de identificare cu variante multiple.

Datele, inclusive rezultatele istoricului si a examenului fizic precum si scorurile obtinute la testele olfactive, au fost analizate folosindu-se o varietate  de proceduri statistice standard.

 

 

Clinical evaluation of olfactory dysfunctions

Abstract

Every year thousands of patients visit ENT and GP offices with chemo-sensory disfunctions. These issues can have a great impact on the quality of life especially for people whose life and safety depend on the well-functioning of their olfactory or gustative senses.

A precise psycho-physical test can be very helpful in identifying and evaluating a chemo-sensory issues. Such a test alongside anamnesis, physical examination, and radio-imagistic exploration can help with the differential diagnostic of the olfactory dysfunction.

The clinical evaluation of olfaction was conducted on a sample of 231 patients who reported various chemo-sensory issues. The routine evaluation of a patient comprised of: anamnesis, full ENT exam, chemo-sensory evaluation, laboratory exams and radiologic exams. In some cases, for establishing the correct etiologic diagnosis, further neurologic, endocrinologic, psychiatric and internist exams were needed. The olfactory test is comprised of two parts: threshold detection and a multiple choice identification test. 

The data, including the results of the anamnesis, of the physical exam and of the olfactory tests, was analyzed using a variety of standard statistical procedures.
